Erection is a hemodynamic process which is based on the relaxation of smooth muscles of cavernous body which are located in its arterioles. During sexual stimulation nitrogen oxide is released out of the nerve endings of activating the enzyme guanylate cyclase. This leads to increased amount of cyclic guanosine monophosphate (cGMP) of the corpus cavernosum. This results is relaxation of smooth muscles of the corpus cavernosum and increases blood flow to the penis.

By blocking specific phosphodiesterase type  5 (PDE5)  Vardenafil drug enhances the local action of endogenous nitric oxide (N0) in the cavernous body during the sexual stimulation. This ultimately causes a strong erection.

After oral administration Vardenafil drug is quickly absorbed and starts working within 15 minutes (in the majority of men after 25 minutes), duration of action is 4-5 hours.

Vardenafil drug contraindications

• Hypersensitivity to any components of the drug.
• Severe cardiovascular diseases
• Severe liver or kidney diseases
• Concomitant use with nitrates or drugs, which are nitric oxide donators.
• The combination of HIV protease inhibitors such as indinavir or ritonavir.
• Vardenafil is not indicated for women and children under 16 years of age.
  

Vardenafil drug should be taken with caution should in patients:
With prolonged QT interval

• With anatomical deformation of the penis (cavernous fibrosis, Peyronie's disease);
• With diseases predisposing to priapism (sickle-cell anemia, multiple myeloma, leukemia);
• With severe hepatic impairment. In Patients with mild hepatic impairment the correction of dose is not required. In patients with moderate hepatic impairment initial dose is 5 mg per day. In future, depending on the efficacy and tolerability of treatment the dose of Vardenafil drug can be increased up to   10 mg;
• With kidney disease

•  With arterial hypotension (systolic blood pressure lower than 90 mm Hg)
•  With the recent stroke and myocardial infarction, with unstable angina;
•  With hereditary degenerative diseases of the retina (eg retinitis pigmentosa);
•  With a tendency to bleeding and exacerbation of peptic ulcer;
•  With aortic stenosis and idiopathic hypertrophic subaortal stenosis.
  

Vardenafil drug Dosing and Administration
The initial dose of Vardenafil drug is 10 mg.
You should take Vardenafil 25-60 minutes before sexual intercourse.
Vardenafil drug's duration of action is 4-5 hours.
The maximum recommended frequency of administration of the drug - 1 times per day. Depending on the efficacy and tolerability of treatment the dose of Vardenafil drug can be increased to 20 mg or decreased to 5 mg per day. The maximum recommended dose is 20 mg 1 time per day.  The dose 5mg of Vardenafil drug is recommended for older patients (over 65 years of age).
The dose 20mg is indicated in severe erectile dysfunction ( in patients with diabetes, prostatectomy), when the beginning dose of 10 mg is not enough effective. Vardenafil drug side effects

Vardenafil drug is generally well tolerated and Vardenafil side effects are usually mild and moderate.
The most observed Vardenafil side effects include (1-10%): headache, facial flushing, indigestion, nausea, dizziness, rhinitis.
Rare Vardenafil side effects include: (0,1-1%): facial swelling, photosensitivity, low back pain, hypertension, changes in functional "liver" tests, increased GGTP, increased levels of creatine kinase, myalgia, drowsiness, dyspnea, blurred vision, watery eyes , priapism, anaphylactic reaction (including laryngeal edema), angina pectoris, hypotension, myocardial ischemia, postural hypotension, syncopal states, increased muscle tone, nasal bleeding, glaucoma.

Vardenafil drug overdose

There is known cases of receiving Vardenafil dose 80 mg per day, which is not accompanied by the development of serious adverse reactions. However, at the same time when used in a dose of 40 mg Vardenafil can cause severe back pain and other most common Vardenafil side effects.

Vardenafil drug interactions

The dose of Vardenafil should not exceed 5 mg when it is used in combination with erythromycin, ketoconazole, itraconazole. The dose of ketoconazole and itraconazole at the same time should not exceed 200 mg. Combination with indinavir and ritonavir is contraindicated.

Nitrate, donator of nitric oxide. Concomitant application of Vardenafil and nitrates is contraindicated because of the risk of arterial hypotension.

A combination of alpha-adrenergic drugs and Vardenafil can be accompanied by the development of arterial hypotension. Vardenafil should be taken no earlier than 6 hours after administration of alpha-adrenergic blockers, and the dose should not exceed 5 mg.
