Augmentin tablets or amoxicillin and clavulanate tablets are used for treating the bacterial infection, therefore is an antibiotic. This antibiotic is used in the medicine for fighting with the respiratory infection, dental infection, urinary tract infection, throat infection and even skin infection. Augmentin is a combination between Amoxicillin and Clavulanic acid, offering a solution against a lot of bacteria and enzymes. 

Action mechanism:

The combination between Amoxicillin and Clavulanic acid is offering a better weapon against the bacteria from the human body. The Amoxicillin is stopping the growth of the bacterial infection in the body by the time Clavulanic acid is inhibiting the enzymes from the human body. Augmentin is the combination between these two antibiotics which are both coming from the antibiotic group named semi synthetic penicillins.

Amoxicillin is a broad spectrum penicillin antibiotic that exerts bactericidal effect against a wide variety of microorganism. Action mechanism is associated with suppressing the synthesis of peptidoglycan in bacterial cell walls during the growth phase of the microorganisms through competitive inhibition of transpeptidase. Clavulanic acid possesses only a weak antibacterial activity, but irreversibly binds to beta-lactamases (forms a stable complex with them), thereby protecting amoxicillin from enzymatic destruction, and provides a guaranteed opportunity to display its antibacterial action. 

Clavulanic acid inhibits beta-lactamase II-V-type classification of Richmond-Sykes. Such enzymes are produced by gram-positive and gram-negative, including anaerobic microorganisms such as Staphylococcus spp., Haemophilus influenzae, Moraxella catarrhalis, Klebsiella spp., Escherichia coli, Neisseria spp., Proteus spp., Yersinia enterocolitica, Bacteroides spp. 

Clavulanic acid is not active against beta-lactamase type I, produced by Enterobacter spp., Pseudomonas aeruginosa, Serratia spp., Acinetobacter spp. 

Clavulanic acid is combined with amoxicillin because the pharmacokinetic parameters of both components are similar and in combination they do not affect the pharmacokinetics of each other. 

Absorption: 
Both components are rapidly absorbed after oral administration. Simultaneous food intake does not affect the absorption. Peak plasma concentrations are reached after about 45 minutes after ingestion. 

Distribution: 
Bioavailability of amoxicillin is approximately 90%. Bioavailability of clavulanic acid is approximately 60%. Amoxicillin and Clavulanic acid bind to plasma proteins by 17-20% and 22-30%, respectively. 

Both components are well distributed in tissues and interstitial fluid - the high concentration of the drug found in plasma, sputum, bronchial secretion, lung tissue, the middle ear, prostate, peritoneal abscess, gall bladder, uterus, ovary, adipose tissue, muscles, bones, skin, biological fluids (including in the synovial, peritoneal, pleural, bile, purulent discharges). 

Metabolism, excretion: 

Amoxicillin is substantially excreted by the kidneys by tubular secretion and glomerular filtration. Clavulanic acid is actively metabolized in the liver and excreted by glomerular filtration and partially in the form of metabolites. Patients with severe renal or hepatic insufficiency require correction of dosing regimen (reduced dosage and/or increase the interval between doses), because clearance is decreased.

Indications:

This antibiotic is used for treating different types of infections or diseases starts such as infections from the respiratory tract, eyes, nose, ears and throat infection. The Augmentin 1mg is used for treating the dental infection such as tooth inflammation or dental abscess. This medication should be kept in a dry place, away from light and kids.

Mode of administration and recommendation of usage:

The most popular Augmentin tablets dose is the 375mg dose which contains 250mg of Amoxicillin and 125mg of Clavulanic acid. However, for those who are having severe infections, there is even a higher dose of 1000mg which contains 875mg of Amoxicillin and 125mg of Clavulanic acid. The Augmentin tablets should be taken only under medical control because it can create an overdose which also brings side effects. The Augmentin tables are taken with a glass of water and the patient should eat before taking this antibiotic. Even if the patient is feeling alright and he doesn’t finish the treatment yet, he doesn’t have to stop taking Augmentin because the infection might appear again.

Contradictions and cautions:

Augmentin is also coming with some cautions. Those who are suffering from hives or asthma should be very careful and they should avoid any treatment based on Augmentin tablets. Also,Augmentin shouldn’t be used for treating the fungi infections or the infections caused by viruses, such as simple season colds.

Side effects:

As any other type of drug, Augmentin can offer side effects too. However, these cases are rarely happening and the usual side effects that can appear are nausea and vomit. Also, in very rare cases Augmentin can bring allergic reactions to different aliments or other medicine.

Contradictions:

The Augmentin shouldn’t be mixed with a lot of other drugs. The ones that are usually avoided by women when they are following an Augmentin treatment are the birth control pills. Also, drugs which are containing aluminum or magnesium should be avoided too.

Drug interactions:

  • Indirect anticoagulants: simultaneous application with amoxicillin may increase the prothrombin time.
  • Antacids containing aluminum, magnesium hydroxide, H2-histamine blockers (cimetidine, ranitidine), M-choline blockers (pirenzepin)
  • Diuretics, probenecid, allopurinol, phenybutasone, NSAID's drugs may supress tubular secretion and increase the concentration of amoxicillin
  • Allopurinol: simultaneous use increases the risk of developing skin rashes, exanthema (the mechanism of this phenomenon is still not clear).
  • Methotrexate: the simultaneous application enhances the toxicity of methotrexate. 
  • Amoxicillin may weaken the effect of oral contraceptives, ethinyl estradiol. 
  • Bactericidal antibiotics (including aminoglycosides, cephalosporins, cycloserine, vancomycin, rifampicin) enhance the effects of Augmentin, bacteriostatic antibiotics (macrolides, chloramphenicol, Lincosamides, tetracyclines, sulfonamides) possess antagonistic effects. Avoid alcohol drinking during the treatment with Augmentin.

Interesting facts:

The maximum amount of Clavulanic acid from all the Augmentin products is 125mg. Also, this antibiotic can be offered to kids too.
